1. Our Pledge Regarding Your Health Information

At CareSage, we understand that your health information is personal. We are committed to protecting it. As a concierge nursing service, we create a record of the advocacy, education, and clinical services you receive to provide quality care and comply with legal requirements.


2. How We May Use and Disclose Your Information

We may use or share your health information for the following purposes:

  • Treatment: To coordinate your care with doctors, specialists (e.g., at Mercy or St. Luke’s), and pharmacies. For example, sharing your current medication list with a rounding physician during an advocacy block.
  • Healthcare Operations: To run our business, improve our masterclasses, and ensure you receive the highest quality of concierge advocacy.
  • Appointment Reminders: To contact you (via text/email) regarding upcoming advocacy appointments or mobile lab draws.
  • To Avert a Serious Threat: If necessary to prevent a serious threat to your health or safety or that of others.


3. Uses That Require Your Written Authorization

For any purpose not listed above, we will ask for your written permission. You may revoke this at any time. We will never sell your information or use it for marketing without your express consent.


4. Your Rights Regarding Your Health Information

  • Right to Access: You may request to see or receive an electronic or paper copy of your clinical notes and records.
  • Right to Amend: If you feel the information we have is incorrect, you may ask us to amend the record.
  • Right to Request Restrictions: You can ask us not to share certain information for treatment or operations. (Note: If you pay for a service 100% out-of-pocket, you have the right to restrict disclosures to your insurance provider).
  • Right to Confidential Communications: You can request that we contact you in a specific way (e.g., only via a specific email or cell phone number).


5. Security of Electronic Communications

CareSage utilizes secure, HIPAA-compliant platforms for texting and emailing. However, please be aware that standard email and SMS may not be fully encrypted. By engaging in these services, you acknowledge the inherent risks of electronic communication.


6. Changes to This Notice

We reserve the right to change this notice. Any changes will apply to all information we already have about you. The current notice will be posted on our website and available upon request.


7. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us directly or with the Secretary of the Department of Health and Human Services. You will not be penalized for filing a complaint.
