3. Complete the Intake Assessment 

The gold standard for concierge nursing is individualized care. Our Comprehensive Intake Assessment is a one-time foundational session required for monthly memberships and select A la carte services.  The Comprehensive Intake Assessment includes:

• Full Physical Assessment: establish a detailed clinical baseline of your current health, allowing me to identify subtle "yellow flags" and customize an advocacy plan that prioritizes your safety and stability. 

• Medication Reconciliation:  a comprehensive review of all your current prescriptions, over-the-counter supplements, and dosages to identify dangerous interactions or dosing errors. This ensures your medication regimen is both safe and effective as you transition between different levels of care.

• Home Safety Assessment: identifies potential environmental hazards and accessibility barriers in your living space to minimize fall risks and ensure your home environment fully supports your recovery and long-term independence. 

While the comprehensive Intake Assessment is the foundational "gold standard" for our monthly memberships and specialized clinical oversight, we believe professional nursing support should be accessible for all needs.
